Privacy of Your Health Care Information

Welcome to the MyUnityPoint Patient Portal

The purpose of this portal is to provide our patients with direct access to information about their health care treatment at UnityPoint Health and to communicate with their UnityPoint Health providers. As we provide you with health care, we maintain records that include information such as your medical history, current conditions, your treatment plan and treatments provided by our health care team, including therapies, surgeries, medications and lab test results. The MyUnityPoint patient portal powered by MyChart is an internet application that enables a patient to have secure web-based access to selected medical information about the patient.

To get a copy of your full medical record, please contact the Health Information Management department of your hospital or the clinic administrator of your local clinic.

To request a medical record correction/amendment, please contact the Privacy Officer of your hospital or the clinic administrator of your local clinic.

Collecting and Using Personal Information

In order to access the MyUnityPoint patient portal, each patient must create an account and login to the website. Once you set up your account, we populate your patient portal account with information about health care services you have received at UnityPoint Health, including your protected health information (such as a name, address, last four digits of your Social Security Number, telephone number, email address, diagnosis, allergies and health history) in the portal for activities such as responding to your questions.

Security of Your Personal Information and Your Responsibilities

When you first enroll to use MyUnityPoint, you will create a MyUnityPoint ID and password. This login process will allow you, or anyone else who has your ID and password, to view your medical information, submit comments that may become part of your record, and communicate with your health care team as if that person were you.

For this reason, it is important that you keep the information you use to log into the MyUnityPoint patient portal completely confidential. You should prevent disclosure of your activation code, ID or password, and you should change your password if you believe that your security may have been compromised. In addition, you should change your password regularly to help maintain the security of your account. You can change your password online at any time by logging into MyUnityPoint and choosing the "Change Password" option.

Direct Messaging

The messaging feature in the patient portal that allows you to communicate with your provider should never be used for urgent matters. The anticipated turnaround time for response to electronic messages is 48-72 hours, Monday - Friday, 8 a.m. to 4:30 p.m. For all urgent medical matters, contact your physician's office by phone, go to an emergency room, or dial 911. If you have a medical emergency, immediately dial 911.

All communications between you and your UnityPoint Health provider using MyUnityPoint are carried over a secure, encrypted connection. While you may receive internet e-mail messages notifying you of new messages in your MyUnityPoint Inbox, these e-mails will not contain any confidential medical information. However, any person with access to the e-mail account to which such messages are sent will be able to see the notifications.


The MyUnityPoint patient portal is a tool to help you manage your health and communicate with your providers. It does not reproduce a patient's entire medical record online, but instead provides access to selected information such as prescriptions, basic laboratory results, summary medical history, and appointments. UnityPoint Health reserves the right to limit health information in the patient portal on a case-by-case basis. For a copy of your full medical record, please contact the Health Information Management department of your hospital or the clinic administrator of your local clinic.

To comply with applicable laws and regulations, UnityPoint Health provides parents and legal guardians with limited proxy access to minors' health information through the MyUnityPoint patient portal. The scope of such access will vary depending on the age and legal status of the minor.

Stored Information

Of the information we learn about you from your visit to the MyUnityPoint patient portal, we store the following: the IP address from which you access the Internet along with the date and time that you access the portal. This information is used to measure the number of visitors to the various sections of our site and identify system performance or problem areas. We also use this information to help us improve features of the site and to make the site more useful.

Linking to Other Sites

From time to time our website may provide links to other websites, not owned or controlled by UnityPoint Health that we think might be useful or of interest to you. We cannot, however, be responsible for the privacy practices used by other website owners or the content or accuracy of those other websites. Links to various outside websites do not constitute or imply endorsement by UnityPoint Health of these websites, any products or services described on these sites, or of any other material contained in them.
